The Gulfport Job Corps Center is soliciting proposals for qualified professionals to provide subcontracted services to students (ages 16-24) for the following position:
Optometrist: Services are to be provided as needed. The Optometrist shall conduct full vision analysis/eye exams for students referred by the Wellness staff (Manager of Wellness, LPN and/or Center Physician) and provide lenses, frames and contact lenses (if clinically indicated) for students. Contract will be awarded for the period of November 1, 2015 to June 30, 2017 with a potential for (3) pre priced option years unless stated otherwise.
Proposals must contain a copy of current Mississippi license, resume and proof of malpractice insurance and proposed hourly rate. Proposals must be received by October 11, 2015. Interested applicants must have access to their own facility. The Center will provide transportation to and from the Optometrist facility.

Job Corps is a National training and employment program administered by the Department of Labor for disadvantaged youth.
