This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. THIS REQUIREMENT IS SUBJECT TO THE AVAILIBILITY OF FUNDS CLAUSE FAR 52.232-18 AND FAR 52.232-19 (APRIL 1984). This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ FA4800-11-Q-MH02. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-46. This is a 100% small business set aside requirement. The associated NAICS code is 711130 with a $7 million size standard. This RFQ contains 06 line items. The line item description is as follows:

CLIN 0001 Regular Service QTY: 53 hours per year
CLIN 0002 Regular Rehearsals QTY: 53 hours per year
CLIN 0003 Fifth Sunday Services QTY: 10 hours per year
CLIN 0004 Fifth Sunday Rehearsals QTY: 10 hours per year

Each option year should be detailed as follows:

CLIN 1001 Regular Service QTY: 53 hours per year
CLIN 1002 Regular Rehearsals QTY: 53 hours per year
CLIN 1003 Fifth Sunday Services QTY: 10 hours per year
CLIN 1004 Fifth Sunday Rehearsals QTY: 10 hours per year

The 2nd , 3rd , 4th Option year Clins should begin with the year it is. i.e. Option year 2 should read 2001 etc.

Service Description:
The Contractor shall provide instrumental music (organist) to assist the congregation in worship for the 1100 Protestant Service at Regimental Memorial Chapel, Fort Eustis. The required Period of Performance is a base year plus four option years starting December 1, 2010.

PROVISIONS/CLAUSES:

Addenda to FAR 52.212-5: The following additional clauses apply to this solicitation: FAR 52.233-2, Service of Protest: (a) Protest, as defined in section 33.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the General Accounting Office (GAO), shall be served on the Contracting Officer TSgt Leland Whipkey (addressed as follows) by obtaining written and dated acknowledgement of receipt from 633'D Contracting Squadron, Attn: SSgt Miranda M. Hikes, 74 Nealy Avenue, Langley AFB, VA 23665-2088. (b) The copy of any protest shall be received in the office designated above within one day of filing a protest with the GAO. EVALUATION OF QUOTATIONS/OFFERS:
Quotes/offers will be evaluated IAW FAR 52.212-2. Evaluation will be a "Best Value" determination with Technical, Past Performance and Price all being factors. Technical and Past Performance will be pass/fail based on compliance with the Statement of Work. Past Performance is deemed relevant and recent if it involves Musical leadership and playing within the past three years. Offerors determined technically acceptable will be asked to perform an audition in front of a panel at the chapel at a later specified date. Specific times will be arranged after receipt of quotation. Required musical pieces and rating criteria for this audition will be given to those judged technically acceptable.
