This is a combined synopsis/solicitation for commercial items prepared
in accordance with the format in Subpart 12.6, as supplemented with
additional information included in this notice. This announcement
constitutes the only solicitation; quotes are being requested and a
written solicitation will not be issued. The solicitation is issued as
Request for Quote (RFQ) for non-personal, “Orthopedic Physician
Services.”
The solicitation is issued as Request for Quotes (RFQ) 75H70520Q00021.
This procurement is Unrestricted under NAICS code 621111 with a
standard business size of $11.0M. The solicitation document and
incorporated provisions and clauses are those in effect through
Federal Acquisition Circular 2020-06, effective 05-06-2020.
Contractor shall provide a firm, fixed-price hourly rate for
Orthopedic Clinic services. The period of performance for this
requirement is 1 Base year plus 4 option years. Estimated start date
is 11 Jul 2020.

The closing date for receipt of quotes is 26 May 26, 2020. Offers
shall be submitted to the Bemidji Area Indian Health Service, 522
Minnesota Ave, Bemidji, MN. 56601, no later than 4:30 p.m. CST, on 26
May 2020.

this office, showing the solicitation number, and your name and

vendors will review the Performance Work Statement and following
information, then submit applicable information. No phone inquiries!
The Indian Health Service (IHS) intends to award a firm-fixed price
purchase order. The Government may make an award without discussion
therefore, the quote should contain the best price and shall be
determined to be the best overall values and most advantageous to the
Government.

The provisions of 52.212-1, Instructions to Offerors-Commercial Items
(Mar 2020), applies to this acquisition. In addition to the
information required in FAR 52.212-1, Contractors shall provide their
