TO PROVIDE FIRE EXTINGUISHER SERVICES, PARTS, AND SUPPLIES, AS NEEDED, TO THE CITY OF BURBANK IN ACCORDANCE WITH ALL TERMS AS STATED IN THE ATTACHED GENERAL CONDITIONS AND BIDDER'S PROPOSAL SHEET. BIDDER MUST RETURN COMPLETED BIDDER'S PROPOSAL SHEET (PAGES 10 THROUGH 12 OF THE GENERAL CONDITIONS). FAILURE TO DO SO WILL DISQUALIFY YOUR BID. SUCCESSFUL BIDDER WILL BE REQUIRED TO FURNISH A CERTIFICATE OF INSURANCE AS STATED IN THE GENERAL CONDITIONS. BIDDER WILL BE REQUIRED TO SUBMIT THE "VENDOR/CONTRACTOR CERTIFICATION" STATING THEY HAVE AN INJURY AND ILLNESS PREVENTION PROGRAM (IIPP) AS REQUIRED BY THE LABOR CODE 6401.7 AND TITLE 8, SECTION 1509 AND 3203 OF THE CALIFORNIA CODE OF REGULATIONS. BIDDER MUST RETURN COMPLETED VENDOR/CONTRACTOR CERTIFICATION FORM (PAGES 13 AND 14) WITH THEIR BID. FAILURE TO DO SO WILL DISQUALIFY YOUR BID.
