1) Type of Notification: Sources Sought

This is a sources sought synopsis only. The purpose of this notice is to gain knowledge of potential qualified sources.

Responses to this notice shall include the following: (a) company name (b) address, (c) point of contact, (d) Federal Supply Schedule (FSS) Number (e) FSS Special Item Number (SIN) (f) FSS Contract Number and (g) geographic region/states covered under contract.

In addition, the respondent must provide a capability statement that addresses the organization's qualifications and ability to perform as a prime contractor the work described below.

2) Description: Personal Services (Ophthalmologist)

The following is a brief summary of the primary duties that the Ophthalmologist must perform. This list is not all inclusive:

This is a part time position for 32 hours per month.

The HCP shall perform a full range of ophthalmologic and specialty care, using government furnished facilities, equipment and supplies. Caseload includes scheduled and unscheduled requirements for care.

The HCP are responsible for diagnostic examinations and the development of comprehensive treatment plans.

Provide treatment within the personnel and equipment capabilities of the medical treatment facility.

The HCP shall refer patients to staff specialists for consultative opinions and continuation of care.

Shall see the patients of other staff healthcare providers who have been referred for consultation and treatment.

The HCP productivity is expected to be comparable to that of other full time ophthalmologists assigned to the same facility and authorized the same scope of practice.

Provide and document medical advice to patients by telephone as appropriate and in accordance with MTF protocols.

Attend periodic meetings of department staff to review and evaluate the care and treatment provided to patients. Review compliance with the MTF quality control program and the standard operating procedures of the MTF. Participate in clinical and quality review processes as determined by Wilford Hall Medical Center policy.

Participate in clinical research as directed by the Ophthalmology Department Chairman. Provide clinical education and instruction to Ophthalmology and optometry students, residents and physician assistant students rotating through the Ophthalmology Service of Wilford Hall Medical Center.

3) Qualifications:

Education. The HCP shall possess a valid Medical Degree from an accredited School of Medicine. Completion of a formal accredited and valid Ophthalmology Residency Training Program.

Continuing Medical Education (CME). Required continuing education for CME shall be at no additional cost to the Government. The HCP shall provide appropriate CME documentation to the QA or designated government representatives within one month after all CME accredited events.

License and Certification Verification. (Refer AFI-44-119, Medical Quality Operations)

The HCP must possess and maintain an active current; valid; and unrestricted license from a US jurisdiction before practicing independently within the defined scope of practice for their specialty.

The HCP shall have and maintain a current Basic Life Support (BLS) certification and Advanced Cardiac Life Support (ACLS) certification.

The Contractor shall perform all primary and annual licensure or certification document verifications for the HCP to include all replacement HCPs not requiring credentialing. The Contractor is responsible for maintaining these certifications in a current status throughout the life of this contract. Verifications shall be submitted to the government at least ten (10) working days prior to the proposed backup/replacement beginning work in accordance with attachment one (1).

The Contractor shall keep their license/registration/certification current. The Contractor shall confirm that the clinical licenses or certifications have not been suspended or revoked by any healthcare facility, public or private, anywhere in the world. This does not preclude any HCPs whose licensure or certification, although originally suspended or revoked by a healthcare facility, was subsequently fully reinstated by the same.

HCP who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. If time exceeds thirty (30) days of abeyance, the provider will be placed in summary suspension IAW paragraph 9.16 until the license, registration or certification is renewed. Issuing agency grace periods are not honored by the Air Force Medical Service (AFMS) when renewing a license, certification, or registration.

HCP who fail to maintain a license, registration or certification will be removed from independent practice and privileged providers will be placed in abeyance. If time exceeds thirty (30) days of abeyance, the provider will be placed in summary suspension IAW AFI 44-119, Medical Quality Operations paragraph 9.16.until the license, registration or certification is renewed. Issuing agency grace periods are not honored by the AFMS when renewing a license, certification, or registration.

Experience. The HCP shall possess at least three (3) years previous experience in an Ophthalmology Department within the last five (5) years.

The HCPs shall have exposure to or experience in clinical research.

Skills and Knowledge. The HCP shall meet skills and knowledge requirements:

Knowledge of clinic regulations, policies, and procedures to receive visitors and patients, screen phone calls, and answer or refer questions.

Knowledge of spelling, grammar, syntax, format, forms, procedures, medical and specialized scientific terminology sufficient to process and type correspondence, provider instructions, and reports.

Knowledge of computerized databases to include office software (i.e. Excel, Word).

Knowledge of the internal organization of patient records in order to search the record to extract medical information and data relevant to patient care.

Knowledge of surgical equipment and procedures to include patient preparation, evaluation, and postoperative care of patients.

The HCPs shall provide special knowledge; skills and professional capabilities in ophthalmologic practice, clinical research and vision care practice management. These include:

All aspects of comprehensive primary care ophthalmology

Preoperative and postoperative management of eye surgery patients

Coordinate with other departments of the medical center staff to provide complete care to patients.

Supervise care provided by military and civilian support personnel.

4) Place of Performance:

These services would be performed at Wilford Hall Medical Center and/or military installations in San Antonio, TX.
