The Nashville Area Office of the Indian Health Service is requesting proposals for Pediatric services at the Catawba Service Unit pediatric wellness clinics. Documents are attached that constitute a request for proposal (RFP) numbered: 285-14-RFP-0013. These pediatric wellness clinics shall take place once a month on a Friday and Saturday.

The standard form (SF-1449) must be completed along with any other signature blocks for consideration. A detailed statement of work (SOW) starts on page three of the RFP document. Please read the SOW section in detail and base your proposal upon the information given therein.

Please fill out all of the required information and submit your proposal back to our office by August 20, 2014. The package shall be mailed to: Donavan Yazzie, 711 Stewarts Ferry Pike, Nashville, TN 37214-2634 by (12:00 pm CST) on August 20, 2014. You may not return the completed documents by email.
