Physical Therapy for MMHI The Wisconsin Department of Health Services (DHS) is seeking bids for Physical Therapy Services, and intends to use the results of this process to award a Contract. The Physical Therapy Services will be located at Mendota Mental Health Institute (MMHI), 301 Troy Drive, Madison, WI 53704. Mendota Mental Health Institute (MMHI) is seeking to replace their expiring contract to provide Physical Therapy Services for their patients. The Physical Therapist functions as a member of the multidisciplinary team and shall, therefore, comply with the appropriate Federal rules and regulations and the Wisconsin Administrative Code. An LPT provides approximately 25-30 hours of therapy services per week. During the 2019 State Fiscal Year (July 1, 2018 – June 30, 2019), MMHI utilized 1,307.50 hours of contracted therapy services.
