This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information attached to this notice.  This announcement constitutes the only solicitation; quotations are being requested at this time. A written solicitation will not be issued. 


The Social Security Administration is seeking quotations for the design and production of plexiglass in accordance with the attached specifications.  The specifications for the plexiglass are included in Attachment No. 1 of this notice.  The small business size standard is 750 employees. The North America Industry Classification System (NAICS) code for this acquisition is 325211.

This acquisition is being conducted in accordance with the Federal Acquisition Regulation (FAR) Part 12, Acquisition of Commercial Items, and FAR Part 13, Simplified Acquisition Procedures.

This requirement is set-aside 100% for small business. Only qualified sellers may submit quotes.

The selected offeror must comply with all attached applicable FAR clauses.  Applicable FAR clauses and provisions include FAR 52.216-18, Ordering, FAR 52.216-19, Order Limitations, and FAR 52.216-21, Requirements, FAR 52.212-1, Instructions to Offerors - Commercial Items, FAR 52.212-4, Contract Terms and Conditions - Commercial Items, FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items.  In addition, offerors must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items.  Applicable terms and conditions, and representations and certifications are contained in Attachment No. 2 of this notice.

To be considered for award, offerors must meet all requirements specified in Attachment No. 1 of this solicitation.  The contractor shall also provide a firm-fixed unit price (completed SF-18) for each contract year provided under this solicitation and state the period of acceptance for their quotation. 

Each quoter must submit one sample product as allowed by FAR Part 52.212-1(d).  A quote will be considered received when the written quotation and sample product are delivered.  All quotes and sample products, based on the specifications, must be received by 4:00pm  EST, May 5, 2011.  Late quotes and samples will not be considered.  All packages delivered to the Office of Acquisition and Grants (OAG) must be signed for.  Please deliver or mail the sample plexiglass to the following address:

The Contracting Officer will evaluate offers on the basis of information furnished by the offeror and the submitted product sample. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.

The Government will award a requirements contract resulting from this solicitation to the responsible offeror whose offer conforms to the requirements of this solicitation in terms of technical acceptability, lowest price and delivery time.  

Shipping/Delivery must be free on board (FOB) destination CONUS (Continental U.S.), 15 days after the issuance of a delivery order. The seller must deliver the plexiglass at the destination specified by the buyer, and the seller is responsible for the cost of shipping/transporting and risk of loss prior to actual delivery at the specified destination. Offerors must be registered in the Central Contractor Registration (CCR) database before an award can be made to them. If the offeror is not registered in the CCR, it may do so through the CCR website at http://www.ccr.gov.

Responses to this combined synopsis/solicitation are due no later than 4 p.m.EST, Thurs. May  5, 2011. Offerors may send responses via email to [email protected].  All questions regarding this RFQ must be submitted in writing to Elizabeth Stewart, at the email address provided previously, by 1:00PM EST on April 29, 2011.   NO PHONE CALLS WILL BE ACCEPTED.  E-mail communications shall include company name, point of contact, e-mail address, phone and fax numbers.
