The Centers for Disease Control and Prevention (CDC) 2920 Brandywine Road, Atlanta, GA 30341 intends to issue a modification to contract 200-2016-89993. The CDC intent is to sole source this modification to the current contractor AVRAM, 1125 NE 125th STE 300, North Miami, Florida 33161.This modification will not change the current period of performance,

The purpose of this modification is to incorporate the following changes:
1. Change the labor in the contract from FFP LOE to FFP.
2. Revise the labor categories and associated pricing for two of the field support officers.
3. Establish a performance based milestone/payment schedule.
4. Add a CLIN for Surge Labor, which will be optionally funded in the event of an outbreak.
5. Change the deliverables schedule for the Transition Public Health Analyst to relative dates.
