The Contractor shall provide all resources necessary to accomplish the tasks and deliverables described in this Statement of Work. The Contractor shall deliver 90ml containers of 10% Neutral Buffered Formalin. Each 90ml of Formalin will be contained in a specimen transport container.
Each specimen transport container will have a label that has the following: name of product, contents that make up the product, appropriate warning labels, emergency contact info, contact for safety data sheet and 5 lines that are labeled and allow room to record - Name, Date, D.O.B., Spec. and Dr.
The size of the container will either be approximately 3-1/4" tall (including the lid) and an approximate base diameter of 2" or approximately 4" tall (including the lid) and an approximate base diameter of 1-11/16".
The specimen transport container will come filled with approximately 45ml of 10% Neutral Buffered Formalin and will have a lid that prevents leaking during shipment. The 90ml containers of 10% Neutral Buffered Formalin will be delivered by the case.
 A case will be comprised of 4 cases of 24. 


The expiration date will be a minimum of 2 years from date of delivery.
