1. Services provided by vendor/contractor/proprietor . The provider will provide the following; * Process of service to individuals designated by the Office of Kentucky Law Enforcement CouncilSupport (OKLEC) * Process of services includes three (3) attempts to serve individuals *Process of service to individuals designated by the Department of Criminal Justice Training (DOCJT) *Process of service includes three (3) attempts to serve individuals *Primary location of vendor business must be within local vicinity where service is requested *Provide last minute service requests within five (5) business daysfrom the process servors receipt of request *Provide change of addressinformation for individuials * An affidavit of service must be provided to OKLEC and DOCJT after service is complete * Items/supplies/equipment provided by provider. The service provide all necessary supplies * Inspection and acceptance of services The agency representative will approve the providors invoice before payment is authorizedInvoice Instructions The vendor is responsible for submitting an invoice to the agency at the address listed above before the 10th of the month following the month in which services rendered.Payments will be made within thirty (30 working days after receipt of goods or a vendor's accurate invoice. * Location of service *Different locals of Kentucky. States of Tennessee, Ohio and Indiana Period of service will be from November 15, 2022, through June 30, 2023. This contract may be extended for one (1) additional year of service
