The Regional Health Contracting Office - Atlantic, Gordon Health Contracting Cell, Fort Gordon, Georgia intends to issue a purchase order pursuant to the authority of FAR 13.106-1(b)(i) on a sole source basis to TMJ Solutions, Inc., 2233 Knoll Drive, Ventura, CA 93003, Oral & Maxillofacial Surgery for a Prosthesis Total Custom Bilateral TMJ Implants Bundle System implant and instrumentation

TMJ Solutions, is the only vendor capable of providing a Prosthesis Total Custom Bilateral TMJ Implants
Bundle System with capability to treat rheumatoid arthritis, each prosthesis is individually fabricated for the
unique anatomy of the patient. She has CT evidence of bilateral fibrous ankylosis of temporomandibular joint,
extensive arthritic changes and subchondral cysts in joint spaces. On physical exam, she has limited mouth
opening (25mm) as well as lateral excursive movements (3mm), causing significant functional limitations.
Prosthesis Total Custom Bilateral TMJ Implants Bundle system has greater modularity options to meet the
Government's need for surgery.

This is not a request for proposal. However, companies that can provide an equal product must submit their technical capabilities to include technical specifications of their product, past performance information, price and delivery time. This information will be reviewed by the Government technical representatives for consideration for future requirements.
