Worker Protection and Investment form BOP-2201 must be completed by an authorized vendor respresentative and uploaded with bid submission. Certificate of insurance in compliance with terms and conditions requirements must be uploaded with bid submission. Current PA small business certification must be uploaded with bid submission.
