SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs, James J. Peters VA Medical Center 130 West Kingsbridge Road, Bronx, New York 10468-3992 is conducting a market survey to identify potential sources to provide autopsy services. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing autopsy services for the Northport VA Medical Center, 79 Middleville Road, Northport, NY 11768. The services will include providing a Forensic Pathologist Level III Medical Examiner to assist the medical center staff pathologist in the performance of autopsies. The service must be available 24 hours a day, 7 days a week. Upon information by the Northport VAMC for autopsy assistance to Customer Service, a representative will respond by phone, for verification, within 15-30 minutes. The contractor will normally be available one to two hours for the autopsy. Should the autopsy be cancelled for whatever reason, the contractor will be notified as soon as possible. If the contractor is to be physically at the hospital before the contractor might be reached the Northport VAMC will be assessed the usual autopsy fee. The duties will consist of assisting the Pathologist as directed, sewing and cleaning of the remains and cleaning the table, instruments and working are used. The contractor will follow C.D.C and O.S.H.A. guidelines and will be trained in all autopsy techniques. The contractor shall check the patient's chart and make sure they have a viable consent before proceeding with the autopsy. Parking facilities will be made available to the contractor while services are performed. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of autopsy services. Sources are asked to provide information to demonstrate their answers to the Following items of information: 1. Company Name, Address, Website, POC, and POC Phone Number. 2. The North American Industry Classification System Code (NAICS) is 621999 and the small business size standard is $14.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3. Provide a summary of experience related to autopsy services described above. 4. If company holds a Federal Supply Schedule (FSS) Contract, please list the GSA Contract Number and relevant SINS.
