Services, Non-Personal - Provide all labor, facilities, equipment, tools, materials, expertise and transportation necessary to provide testing services for blood specimens provided by Keesler Medical Center as required for Government beneficiaries at Keesler Air Force Base (KAFB), hereafter referred to as Keesler Donor Center. Performance shall be according to this Performance Work Statement (PWS) and the professional standards, guidelines, and regulations of the American Association of Blood Banks (AABB), the Food and Drug Administration (FDA), and the Joint Commission on Accreditation of Healthcare Organizations (JCAHO). As an accredited and affiliated member of the Armed Services Blood Program (ASBP), responsible for supplying blood to soldiers, sailors, airmen and marines deployed worldwide, the Keesler Blood Bank conducts frequent blood drives. Specimens of each donated unit must be tested by an independent agency. The Contractor shall provide shipping containers for specimens to be tested under Enzyme-linked Immunosorbent Assay (EIA) protocol and Investigational New Drug protocols (i.e. West Nile Virus (WNV), single donor Nucleic Acid Testing (NAT) as required by the FDA. The Contractor shall provide transportation from the Keesler Donor Center in an adequate amount of time to ensure results are transmitted via File-Transfer-Protocol (FTP) to the Keesler Medical Center no later than 1:00 PM the following day (Fax may be used in extreme circumstances). The requirement is UNRESTRICTED with a NAICS code of 621511 and a small business size standard of $9 million. The period of performance is base period of a tentative date of 1 Feb 07 ? 30 Sep 07, plus 4 one-year option periods. The place of performance is unknown, however, the Contracting Officer has requested wage determinations for Mobile, AL, Pensacola, FL and New Orleans, LA as the most likely places of performance. Wage determinations for additional possible places of performance will be requested if the request is received in writing (fax and email OK) prior to the release of the solicitation tentative date of 6 Dec 06. All responsible sources may submit a quote and at least 2 references, which will be considered by this agency. This request for quote is issued under Simplified Acquisition Procedures (SAP) and award will be made in the form of a firm-fixed price requirements contract.
