Company Name, Address, Phone Number, Point of Contact Three Reference Points of Contact for similar work Previous Hospital/Medical Clinic Project Experience within last three years The Contractor will receive an ultrasonic evaluation of the patients prostate gland and the ultrasound prostate implant prescription from the VA Sierra Nevada Health Care System, Reno, NV, Urology Department. Using this information provided by the VA, Reno, and the contractor will determine and provide a written description of the required number and pattern of Brachytherapy seeds for placement in the prostatic bed and the dose of radioactivity per seed. Contractor shall provide a Dose Plan within Four Working Days of the receipt of all necessary supportive documentations from the VA, Reno. This information will then be given to the Urology Physician Team located at the VA, Reno.
