The Department of Veterans Affairs Medical Center, Medical Center, 10 Casia Street, San Juan, PR has a need to procure Radiation Physician Therapy Services for Brachytherapy Services. The solicitation package will be available on or about November 21, 2002 by written request only. Request must be faxed to Elisa Cruz, Contracting Officer at (787) 641-8364 or via e-mail [email protected]. The contract period will be for one base period with 2 option years period. The following procedures are included as part of the services (where needed) Therapeutic Radiology Simulation-Aided Field Setting =96 Complex (CPT Code 77290), Basic Radiation Dosimetry calculation, central axis depth dose, TDF, NSD, gap calculation, off axis factor, tissue inhomogenity factors, as required during course of treatment, only when prescribed by the treating physician (CPT Code 77300), Brachytherapy isodose calculation, intermediate (multi-plane dosage calculations, application involving 5 to 10 sources/ribbons, remote after loading brachytherapy, 9 to 12 sources (CPT Code 77327), Continuing medical radiation physics consultation including assessment of treatment parameters, quality assurance of dose delivery and review of patient treatment documentation on support of the radiation oncologist reported per week of therapy (CPT Code 77336), Special medical radiation physics consultation (CPT Code 77370), Special Treatment Procedure (CPT Code 77470), Remote after loading high intensity brachytherapy, source positions or catheters (CPT Code 77783), Supervision, handling and loading of radio-element (CPT Code 77790), Office Consultation for a new or established patient which requires these three key components: a problem focused history; a problem focused examination and straight forward medical decision making. Usually the presenting problem(s) are self limited or minor. Physicians typically spend 15 minutes face to face with the patient and/or family (CPT Code 99241).
