1. What is the average length of time for credentialing at your facility? (Answer) Three (3) months 2. Will you consider a non-boarded Mohs physician who has VA experience? (Answer) No 3. What is the Work Schedule on Tuesdays? I noticed that the schedule doesn't cover that day and the hours don't total 40 per week? (Answer) Schedule pending; four days. 4. Is a DEA required. (Answer) Yes 5. Can you please clarify on the cases/days? Does the workload consist of 4-8 MOHS cases on top of seeing clinical patients after that (the 5-8 new patients and 4-10 return patients)? Or just the 4-8 MOHS per day? What is the total patients/day we're looking at? (Answer) 4-10; profiles aren't set yet. 6. How many patients per day are expected? (Answer) Profiles aren't set yet; up to 30. 7. How many surgeries? (Answer) Depends on what the surgeon finds. 8. Confirming this is outpatient only with no call or weekends? (Answer) Outpatient only 9. Are there other staff DERM's they will be working with?(Answer) No 10. What is the mid-level support? (Answer) None 11. Will you please elaborate on training of residents as stated in scope of services? What does the entail? (Answer) Not at this time. Fellows are trained; not residents. 12. Is rate to include travel and lodging? (Answer) No 13. What is the minimum length of commitment you will accept? 3 months / 6 months etc? (Answer) Six-months.
