Modification to Sources Sought originally posted February 6, 2017 Clarification: the response date is February 14, 2017 not April 21, 2017 William S. Middleton VA Hospital (VAH) located at 2500 Overlook Terrace, Madison, WI 53705 William S. Middleton VA Hospital (VAH) is seeking vendors to provide Hemodialysis and Continuous Renal Replacement Therapy (CRRT) to Veterans 24 hours, 7 days a week, 365 days per year to include holidays at the VA Hospital. The vendor shall provide a pool of qualified Registered Nurses, all dialysis equipment, dialysis supplies and pharmaceuticals. Vendor shall provide a pool of qualified dialysis, Registered Nurses. (VA considers a pool of at least 5 persons to meet the VA requirement of 24/7/365). Vendor must submit qualified Registered Nurses to the VA Contracting Office for review and evaluation within 2 weeks once a contract has been awarded. The prospective Registered Nurses shall successfully be adjudicated to a mandatory National Agency Check and Inquires (NACI) background investigation prior to performing any services at the Madison VA Hospital. Once the background investigation has been submitted, the Madison VA will begin the credentialing and privileging process.  Registered Nurses cannot be scheduled to work prior to being granted privileges. Vendor shall transport all dialysis equipment, dialysis supplies, and pharmaceuticals to and from the VA Hospital as required. The VA hospital does not have storage space to store equipment or supplies. All dialysis equipment shall be maintained and repaired to optimal operating condition in accordance with industry standards, at the vendor s facility. Equipment shall be clean and sterilized prior to any treatment. All supplies and pharmaceuticals shall be current and not out of date. Vendor shall perform bacterial and chemical water testing in accordance with the Joint Commission and AAMI industry standards and guidelines. Vendor shall supply all dialysis pharmaceuticals for the Hemodialysis treatments and the CRRT. Vendor shall be responsible for the composition of the dialysate, administering IV solutions, blood and blood products, dressing changes, suture removal, bandages, and medications required for dialysis treatment. Hemodialysis Routine Treatments: vendor shall to perform treatments within six (6) hours or timeframe determined by VA Nephrologist after receiving notification by VA for non-emergent requests Hemodialysis Emergency/Stat Treatments: vendor shall provide emergency treatments no later than two (2) hours after the time they are notified. CRRT treatments shall be provided within eight (8) hours after receiving notification by VA. Where warranted, based on the seriousness of the patient s condition, vendor shall respond within two (2) hours of contact by VA. Vendor shall document in the patient s VA medical record when additional items (i.e. medications or lab tests) are required for that treatment session. Vendor will not be allowed to invoice for any services not documented in the patient s medical record forwarded to the VA. Applicable NAICS Code is 621492. This procurement will be a Firm-Fixed (unit) Price, Indefinite Delivery, Indefinite Quantity contract for a five year ordering period, from date of award. This Sources Sought shall not be considered as a request for proposal or as an obligation on the part of the Government to acquire any products or services. No entitlement to payment of direct or indirect costs or charges by the Government will arise as a result of the contractor submission of this Sources Sought or the Government's use of such information. The Government reserves the right to reject, in whole or in part, any contractor's input resulting from this Sources Sought. No contract will be awarded as a result of this announcement. Data submitted in response to this Sources Sought will not be returned. All information should be UNCLASSIFIED material only.
