The William S. Middleton Memorial VA Hospital, 2500 Overlook Terrace, Madison, WI is seeking potential sources capable of providing all personnel, equipment, supplies and pharmaceuticals to provide hemodialysis, peritoneal dialysis, continuous renal replacement therapy (CRRT), and plasmapheresis on an as-needed basis, 24/7, 365 days a year to include federal holidays. All treatments will be performed at the VA hospital. The applicable NAICS code is 621492 with a size standard of $38.5 million. Historical information for the period of May 2014-May 2015 is provided below. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned (SDVOSB); firm's experience in providing the listed service; answers to the below questions, and any other pertinent information that demonstrates the capability to meet the above requirement. QUESTIONS " Can your company provide a pool of nurses to provide hemodialysis, peritoneal dialysis, CRRT, and plasmapheresis (approximately 4-5 nurses)? " Does your company offer these services to inpatient basis (i.e. at the patient's bedside) or must the patient come to your facility? " If your company offers these services to inpatients, does the company provide its own equipment? If yes, is a store room required or can you bring the equipment in for each treatment? " What is the typical timeframe for treatment services after notification of request? Is it possible to provide services on a STATS basis within two hours of the request? " How is a typical dialysis treatment defined, ?4 hours? " What is industry standard with concern to line item structure for billing? How are CRRT services priced (i.e. setup, maintenance, reset)? " Would your company be willing to submit its personnel to VA background investigations and credentialing requirements?
