The Purpose of this sources sought announcement is for market research to make appropriate acquisition decisions and to gain knowledge of potential qualified Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, 8(a), HUBZone and other Small Businesses interested and capable of providing the services described below.
Documentation of technical expertise must be presented in sufficient detail for the Government to determine that your company possesses the necessary functional area expertise and experience to compete for this acquisition.

Draft Performance Work Statement:
The purpose of this sources sought announcement is for the Grand Junction Veterans HealthCare System to obtain offers from contractors who can provide Off-site Radio Nucleotide Treadmill Stress Tests and Pharmacological Stress Studies Services to Veterans in Grand Junction, Colorado. The services shall be performed at the contractor's business location and the contractors business shall be located within a 25 mile radius of the Grand Junction Veterans HealthCare System, 2121 North Ave., Grand Junction, CO 81501, and must comply with the Americans with Disabilities Act.
The Contractor shall furnish all personnel to provide services necessary to perform Radio Nucleotide Treadmill Stress Tests and Pharmacological Stress Studies to eligible beneficiaries of the Grand Junction Veterans HealthCare System. The Contractor shall provide an interpretation for each test conducted. The contract physician(s) and nuclear medicine technologist(s) care shall cover the range of services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be a quality, meeting or exceeding currently recognized national standards as established by the American Board of Internal Medicine in cardiology (http://www.abim.org).
Inpatient Cardiologic Consultations shall not be provided under this requirement.
The Off-Site Radio Nucleotide Treadmill Stress Tests and Pharmacological Stress Studies Services will be required on an as needed per procedure basis. Only the services listed by Current Procedural Terminology (CPT) codes below may be provided and billed for against the resulting contract.

Qualifications:
License - Contract Physician(s) and Nuclear Medicine Technologist(s) assigned by the Contractor to perform the services covered by this contract shall have a current to practice medicine in the State of Colorado, and shall be full and unrestricted licenses.
Board Certification - The contract Physician(s) shall be board certified by the American Board of Internal Medicine in Cardiology (www.abim.org), the Nuclear Medicine Technologist(s) shall be registered technologist, Radiology, Nuclear Medicine - R.T. (N)(ARRT) or Nuclear Medicine Technology Certification Board (NMTCB). The contract Physician(s) and Nuclear Medicine Technologist(s) shall be currently certified in Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS) or equivalency.
