The Federal Bureau of Prisons, Field Acquisition Office, Grand Prairie, Texas, intends to issue solicitation RFQ 41207-001-6 for the provision of providing Optometry Services at the Federal Medical Center, Rochester, Minnesota. The contractor must provide optometry services to patients in a correctional setting. The optometrist must be a graduate of an approved school of optometry and possess a valid and current license to practice optometry in any one of the states or territories in the United States. Must possess an expanded optometry license that permits the prescription of legend ophthalmologic medications and diagnoses and treatment of glaucoma. It is estimated the contract will include 52 weeks of dental assistant services @ 8 hours per week per contract year. A session will be defined as one hour. Estimated total sessions per year is 416. The contract period shall consist of one 12-month base year and four 12-month option periods for renewal. The solicitation will be available on or about June 10, 2005, and will be distributed solely through the website at www.fedbizopps.gov. All future information concerning the acquisition, including solicitation amendments will be distributed through the website. The anticipated receipt of quotes is July 12, 2005. All responsible small business concerns may submit a quote which will be considered. Faith-Based and Community-Based Organizations have the right to submit quotes equally with other organizations for contracts for which they are eligible.
