Wylie VA Ambulatory Care Center (VAACC) in Columbus, Ohio has a requirement for Otolaryngology (ENT) Services to provide service for the Center's patients within the Greater Columbus, Ohio area. The Medical Center will be seeking to solicit a Firm Fixed Price contract in the near future. Interested parties should provide the following: Company Name, Company Address, Point of Contact, (POC) phone number/email address, DUNS number and capability statement for the services as stated above. Responses to this notice should include three customer references to which you have provided similar services to, copies of applicable licensing, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business?
