This pre-solicitation notice precedes the posting of a solicitation for the anticipated award of a firm-fixed price, indefinite-delivery indefinite quantity (IDIQ) 5-year (base year plus four (4) one (1) year option periods to be exercised at the discretion of the Government) CPT-coded procedures contract for: Pathology Laboratory Services. It is the intent of the New Mexico Veterans Affairs Health Care System (NMVAHCS), facility located at 1501 San Pedro Dr SE, Albuquerque, NM 87108, to solicit and award such a contract on behalf of the West Texas Veterans Affairs Health Care System, 300 Veterans Blvd, Big Spring, Texas, 79720.
In accordance with this contract, contractor shall provide CPT coded procedures to include but not be limited to:

In providing the above listed procedure the following is a description of the services that will be required in accordance with the solicitation:
Since these services are not provided by the WTVAHCS Pathology Department, the WTVAHCS requires subject services as an adjunct in providing the maximum care available for eligible veteran beneficiaries. The services shall be conducted following consultation with the WTVAHCS Pathology/Laboratory Program Office and in accordance with the requirements of this Performance Work Statement.
SUMMARY OF REQUIREMENTS
Contractor shall provide all supervision, administrative and technical support, labor, subcontractors, equipment, supplies, transportation, and material for the following: examination, testing evaluation, reporting of autopsy, and cytology results of the WTVAHCS furnished specimens.
Contractor shall provide Emergency Services involving: receipt of wet tissue specimens in cassettes and embedding and cutting slides. Advance notification and specifics regarding these situations will be provided.
Contractor shall provide collection and transportation of supplies, i.e., prepared cytolyte and thin prep collection kits and specimens. The WTVAHCS reserves the right to forward specimens to Contractor, and the right to request that the contractor provide the WTVAHCS with a second opinion. Second opinion services shall be conducted at the contractor's facility. Contractor shall pick up specimens (contractor shall package) from the WTVAHCS Medical Center Monday through Friday between 10:00am to 12:00pm, and maintain proper specimen storage temperatures throughout the transition from the Medical Center to contractor's facility. The contractor shall examine and evaluate specimens at Contractor's facility.
If a second opinion is required and/or referred by the contractor to an outside vendor, the contractor is financially responsible for the services rendered; the WTVAHCS shall not be invoiced for such services.
Results of examination and evaluation shall be transmitted via telephone when requested, otherwise on WTVAHCS Medical Center provided forms that shall be delivered by contractor's courier to WTVAHCS Pathology Laboratory Service. If electronic transmission is established, that method of transmitting shall be used when appropriate at no additional cost to the Government.
Results of Tests:
A report is defined as: a printed final copy in triplicate of Laboratory testing results. If results are telephoned, then the report must include the name of the individual notified of the results.
Each test report shall provide at a minimum the following information:
Patient's name and/or identification code
Physician's name (if supplied)
Medical record number (if supplied)
Facility Name
Patient's location (clinic/ward)(if supplied)
Date/time specimen received in reference laboratory (contractor and/or subcontractor)
Test ordered
Date/time of specimen collection (when available)
Date test completed
Test results with abnormalities flagged
Reference range
Toxic and/or therapeutic range where applicable
Testing laboratory specimen number
Name of testing laboratory (contractor and/or subcontractor)
Type of specimen
Any additional comments related to test, provided by submitting labs
Any information the laboratory has that may indicate questionable validity of test results
Unsatisfactory specimen shall be reported with regard to its unsuitability for testing.
Routine case(s): report response time is required within 48 hours from the time of pick up (surgical cases requiring special staining response time is required within 72 hours). Second opinions required under this contract shall be furnished within five (5) work days of request.
Contractor shall offer consultations if requested by the Chief of Pathology and Laboratory services. On-site consultation visits to the contractor's facility may be required.
PERFORMANCE STANDARDS
As defined in the Quality Assurance Surveillance Plan (QASP), the following Indicators are the services that will be monitored by the WTVAHCS:
Reporting requirements following routine services and testing
Reporting (verbal and written) following urgent services and testing
Accreditation and renewals
Credentialing and renewals
Reporting requirements of patient safety incidents
Investigation, confirmation and resolution of patient safety incidents
Continuous reporting requirements of patient safety incidents
Availability of pathologist(s) for consultation when needed
In accordance with the requirements of 10 U.S.C. 2304 and 41 U.S.C. 253 and as per FAR 6.401(b), this procurement will be conducted using full and open competition procedures.
FAR part 12, Commercial Items, and part 15, Contracting by Negotiation apply. NAICS code is 621511, $13.5 million. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The procurement is subject to the availability of funds.
