The Department of Veterans Affairs, VA Medical Center, 215 North Main St., White River Junction, VT 05009-0001, intends to negotiate a sole source procurement for the Contractor to provide laboratory, clinical and pathology testing services to the Department of Veterans Affairs at White River Junction, VT. Such services shall include, but not be limited to, the following: 1. Contractor will provide laboratory testing services pursuant to a pre-determined price list (fee schedule) agreeable to the VA for samples delivered to it by the VA. 2. Contractor will provide an agreed upon testing service manual, acceptable to the VA, which includes test descriptions, CPT codes, handling and storage requirements, maximum turnaround times, assay schedule and method, etc. 3. Contractor will have the following accreditations: CAP, CLIA, and the director shall be a licensed American Board of pathology certified pathologist. 4. Contractor will comply with VA and Federal HIPPA rules and regulations. 5. Contractor shall insure procedures are current, acceptable, accurate and precise, with quality control acceptable to the VA, and shall give prior notice to the VA of any changes in important components of an assay, and the efficacy of the test must remain unchanged or be improved. 6. Contractor shall insure that all employees have appropriate and current licenses, and meet qualifications required by CLIA '88 guidelines. 7. Contractor shall provide results promptly to VA, and in a manner and timetable acceptable to the VA, and to no other person or entity. 8. Results will be reported to the VA twice per day, and STAT results shall be reported immediately after the test is performed, and shall contain information designated by and acceptable to the VA. 9. Contractor shall store specimens a minimum of seven days. 10. Contractor's laboratory shall be geographically proximate to this VA's location in order that transportation of samples and results be made both timely and at an acceptably low cost. 11.
