The VA Healthcare Network Upstate New York at Syracuse, a division of the Department of Veterans Affairs (VA) has a need for specialized medical resources to be furnished by the Contractor. The specialized medical resources required are chronic hemodialysis services to eligible beneficiaries. The contractor will be responsible to furnish medical facilities, clinical providers, equipment supplies and administrative functions to fulfill the level of care specified in the RFP. Service locations are needed in the following geographic locations ââ¬â Onondaga county, Oswego county, Cayuga County, Broome county, St. Lawrence county, Cortland/Tompkins county, Oneida county and Jefferson county. Service will be at an all-inclusive rate. It is the intent of the VA to award a one (1) year contract with four (4) one-year options. This is commercial item acquisition and will be conducted in accordance with policies and procedures of FAR Part 12. The NAIC Code is 621492 and is unrestricted. The Request for Proposal (RFP) will be issued on or about May 9, 2005 and offers will be due as specified in the solicitation, anticipated to be on or about June 9, 2005 for services to begin approximately August 1, 2005.
