Provide certified ophthalmologist who has had at least one (1) year of fellowship in retinal diseases, with current licensure to perform ophthalmology services at the Contractor's establishment for beneficiaries of the VA Medical Center, Augusta, Georgia. The following ophthalmology exam by CPT Codes are to be performed: 92002, 92004, 92012, 92014, 92225, 92226, 99024, 92250, 92235, 76512, 92135, 92270, 92275, 92083, 67210, 67227. Contractor shall be within a 25-mile radius of the VA Medical Center, Augusta, Georgia. Contract period is to be January 1, 2005 through December 31, 2005, with four (4) one-year options to extend. This procurement is being conducted under FAR Part 12 and FAR Part 14. The solicitation is being issued as an Invitation for Bids. The NAICS Code is 621320 and the size standard is $6.0 million. This procurement is 100% set-aside for small business concerns. The anticipated issue date is November 26, 2004.
