The Department of Veterans Affairs, New Mexico VA Health Care System (NMVAHCS), located in Albuquerque, New Mexico, requires a contractor to provide BRCAnalysis (CPT codes 83891x1, 83898x81, 83904x81, 83909x81, 83912x1) testing for hereditary breast and/or ovarian cancer. The Government intends to enter into contract, on a sole source basis, with Myriad Genetics, Inc., 320 Wakara Way, Salt Lake City, UT 84108-1214. The North American Industry Classification System (NAICS) is 334516, Analytical Laboratory Instrument Manufacturing. Any firms who wish to identify their interest and capability to provide the above listed services must provide written notice to the Contract Specialist no later than 3:00 p.m. CDT, 05/29/2013. Such notification must include any documentation which shall contain certifications and descriptions of how the firm will meet or exceed the Government's minimum needs.
