DESCRIPTION: The Northern Arizona VA Health Care System (NAVAHCS), 500 US Highway 89 North, Prescott, AZ 86313-5000 under the authority of 38 U.S.C. 8153 is soliciting information from responsible contractors that provide clinical laboratory pathology services in the catchment area served by NAVAHCS. The VA is conducting a market survey to determine the availability and market pricing from businesses capable of and interested in providing laboratory pathology service to include clinical laboratory management services, anatomic pathology services according to the current years APC or Part B Medicare Schedule for beneficiaries of the Department of Veterans Affairs and enrolled patients of the NAVAHCS. The catchment area is defined according to VA standards in which NAVAHCS patients will not be expected to drive more than 20 minutes. In a manner of speaking the catchment area shall include all of Prescott, Prescott Valley and outlying areas, please view map below for further clarification.
The North American Industrial Classification (NAICS) Code is 622110. Contract period shall be a base year with four (4) option years when awarded. Please express interest and capability by responding to this Sources Sought Announcement no later than 4:00 p.m. Eastern Standard Time (EST) on January 20, 2010. All prospective contractors are required to have an active registration in the Central Contractor Registration (CCR) and FAR Part 4.12, prospective contractors must complete electronic on line annual representations and certification (ORCA) databases prior to award of a contract.
