STATEMENT OF WORK A. GENERAL INFORMATION 1. Transcription Services 2. Scope of Work: The following transcription services will be provided by vendor to the Pathology & Laboratory Medicine Service, Brooklyn Campus, NY, 11209. " All Surgical Pathology Patient Report(s) will be transcribed by the company " All Cytopathology Patient Report(s) will be transcribed by the company " All Autopsy Patient Report(s) will be transcribed by the company The Surgical Pathology, and Cytology, Laboratories, process approximately 10,000 specimens per annum. The pathologist's interpretation is transmitted to the patient's providers by way several written report(s), dictated by pathologist and transcribed by the transcription service. a) Preliminary gross description b) Microscope interpretation c) Final report* " Subject to supplemental finding, i.e., special stains professional consultations. Etc. Transcription Services will be provided for all dictations of Surgical, Cytology, and Autopsy patient reports, submitted to the contracted transcription service. The submitted dictations consist of, but are not limited to, gross description(s); Microscopic interpretation' final report(s); and supplemental reports. All changes will be per line of transcription. Report Turn Around Time (TAT) Transcribed reports are to receive from the transcription service, by Pathology and Laboratory Medicine Service-Brooklyn VA, within twelve (12) hours. Reports will be sent to Pathology & Laboratory Medicine Service-Brooklyn via a dedicated printer set-up with the vendor. Equipment: (Own by VA) Item one: Dictaphone hand free model Item two: Hand Microphone with cradle Equipment provide by: DICTAPHONE Corp. A Pitney Bowes Co. New York, NY 10022 Payment for services will be requested on a yearly 2237 with one Purchase Order number for the stated fiscal year. 3. Performance Period: This agreement shall be entered for a term of nine (9) months from the date of the agreement. 4.
