I. PURPOSE: The purpose of this announcement is to gain knowledge of potential qualified sources and their size classification (hub-zone, 8(a), small, small-disadvantaged, service-disabled Veteran-owned small business, Veteran-owned small business or large business) relative to NAICS Code 621610 (size standard of $16.5 million) and product service code (PSC) Q702. Responses to this notice will be used by the Government to make appropriate acquisition decisions. All interested offerors will have to respond to this notice in order to be considered.

II. BASIC REQUIREMENT: The Department of Veteran Affairs, Network Contracting Office, VISN 9, will award multiple contracts to provide Durable Medical Equipment (DME) Services for six different VA Hospital Systems in Tennessee (TN) and Kentucky (KY).The intended contract period will be for a Base and four one-year options.

The contractor shall provide all supplies, materials, equipment, labor, supervision, transportation, infection control, storage, cleaning, delivery, sanitization, assembly/set-up, repair, pick-up and patient training/instruction-in-the-use of Government-owned DME.

The contractor must have an inventory tracking system in-place before contract award and be available for short notice emergency deliveries. Some Government-owned DME will be required to be separately-stored, assessed for repairs or disposal, cleaned and sanitized, and subject for re-issue from the contractor s premises to the Veteran-beneficiary s home. This would require a properly licensed and established storage/warehouse located within the serviced VAMC.

The government has no intention of buying any DME under this contract.

The DME service is prescription-driven providing the following items (list is not all inclusive) to Veteran beneficiaries: Manual or electric hospital beds, regular or specialty mattresses, manual or power lifts (does not include vehicle-type), scooters, basic wheelchairs (not custom), walkers, etc.

The contractor shall develop mechanisms and methods to register complaints, resolve emergencies, issue approval of Patient Rights and Responsibilities, and provide pertinent documentation of performing all tasks as identified herein.

III. SERVICE AREA: The patients under this contract are served by VISN 9 VA Medical Centers in Louisville, KY, Lexington, KY, Mt Home, TN, Memphis TN, and Tennessee Valley Health Care consisting of Nashville & Murfreesboro, 24-hours a day, seven days a week, including holidays.

The area of service shall be all patients serviced by the VISN 9 VA Medical Centers, including surrounding areas, regardless of where patient resides. Should a Veteran(s) be located within a zone that is not listed for that particular VA Hospital, but is within a zone listed through a separate VA Hospital, the Government reserves the right to purchase the services from the contractor awarded the contract for the zone that the Veteran resides in.

I have ten cargo vans capable of storing and delivering DME products within a 100-mile radius. We are a 40-employee organization with certification from The Joint Commission (TJC) standards for suppliers of DME and so on.
