This is a solicitation for commercial services prepared in accordance with the format in FAR part 12, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested. Solicitation Number VA250-13-R-0770 is being issued as a Request for Proposal with the intent to award a firm fixed price contract for services. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. This is an unrestricted solicitation. Preference will be given to service-disabled, veteran-owned small businesses and veteran-owned small businesses. This is a solicitation for a multi-year proposal consisting of a base year plus four (4) option years. SCOPE: The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (LSCDVAMC) intends to procure radiation dosimetry service for the Louis Stokes Cleveland VA Medical Center, located at 10701 East Blvd., Cleveland, OH 44106. Service shall be in accordance with the terms, conditions, provisions and schedule of the contract, and shall also be in compliance with 10 CFR 20 and Chapter 3748 of the Ohio Department of Health Ohio Revised Code. Bidders should thoroughly review the specifications and be familiar with the area of coverage prior to submitting their bids in order to be fully aware of the supplies and/or services required. Failure to do so will not relieve the winning bidder from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Government. Statement of Work for Dosimetry Services Background Requirement Failure to maintain an appropriate dosimetry program could result in lawsuits, fines, and/or the revocation of the Louis Stokes Cleveland VA Medical Center (LSCVAMC) permit to use radioactive material under the Department of Veterans Affairs Master Materials Licenses issued by the US Nuclear Regulatory Commission. Maintaining a radiation dosimetry program requires a dosimetry vender accredited by the National Voluntary Laboratory Accreditation Program (NVLAP) for the radiation types and energies used at the LSCVAMC. The vender will provide radiation dosimeters for measuring occupational radiation dose from medical X-rays and radioactive material (nuclear medicine). Introduction This Statement of Work provides the specifications for a vender contract to provide radiation dosimeters and dose reports for measuring occupational radiation at LSCVAMC. All vendor transactions involving radiation dosimeter additions, deletions, changes or inquiries regarding occupational radiation dose shall be with the LSCVAMC Radiation Safety Officer (RSO) or RSO designee. Deliverables The vendor will provide occupational radiation dose reports and the number of radiation dosimeters specified by the RSO. The selected vendor shall demonstrate their capability of providing radiation dosimeters (badges) and radiation dose reports specified within this Statement of Work; including demonstrating proof of NAVLAP accreditation. The vendor will maintain permanent occupational dose records for monitored workers and provide LSCVAMC with dose reports as specified by US NRC Regulatory Guide 8.34, Monitoring Criteria and Methods to Calculate Occupational Radiation Doses. The successful vender shall provide representative samples of dose summary reports & annual occupational dose reports (i.e., NRC Form 5 or equivalent). Badges Presently the LSCVAMC uses approximately 250 radiation dosimeters in two accounts for measuring dose from radioactive material & medical X-rays. o Radioactive material account (nuclear medicine & research) - 3 badge series with approximately 30 whole body dosimeters and 15 finger ring extremity badges. o X-ray account (diagnostic X-rays & radiation oncology) - 11 series with approximately 200 whole body dosimeters. Single badge (collar) Effective Dose Equivalent (EDE) calculations per US NRC Regulatory Guide 8.40, Methods for Measuring Effective Dose Equivalent from External Exposure. o Area Monitor - Approximately 10 dosimeters used for room monitors (non-occupational) o Fetal Badges - Approximately 2 whole body dosimeters assigned to declared pregnant workers for monitoring their fetal dose. o Extremity Finger Rings - Approximately 15 finger ring dosimeters provided for nuclear medicine workers to monitor their extremity dose. o Control Badges - Approximately 6 whole body dosimeters and 2 finger rings used for background radiation subtraction and quality control. The RSO will establish badge series for each LSCVAMC Service (department) with workers requiring radiation dosimeters. Each series will have a unique alphanumeric series code. Dosimeters shall be color coded to identify the wear period, and have distinct markings to aid in proper wear positioning. Dosimeter colors shall alternate monthly with each wear period. Finger rings should come in at least three sizes, have a durable/waterproof label and be capable of withstanding cold sterilization. The ring badges shall be color coded for alternate months. Body badges shall be durable, waterproof and labeled with the badge series, badge type, worker name and wear period. Laboratory Accreditation The vendor shall maintain accredited by the National Voluntary Laboratory Accreditation Program (NVLAP) of the US National Institute of Standards and Technology for all radiation testing categories. X-ray <100 KeV photons 100 KeV - 250 KeV photons Isotopes > 250 KeV up to and including Cs 137, Co 60 Photons from a 6/18 MeV external beam linear accelerator Beta radiations Badge Shipments Dosimeters shall be received by the RSO before the 20th calendar day of each month. Wear period is for each calendar month, and badges are exchanged on the first day of the month. Expedited order dosimeter(s) shall be shipped next day delivery, and received by the RSO within 2 working days. Radiation dosimeters shall be shipped to the RSO in presorted envelopes labeled for each series. Shipments shall include printed packing list for each account and badge series. The packing list shall include the wear period, each badged worker's name and badge type. Customer Service The vendor shall be accessible during regular working hours and ensure timely response to RSO inquiries. The RSO or designee shall be notified by telephone or email of all readings above the notification levels set by the RSO. The vendor shall expedite processing of radiation dosimeters when requested by the RSO. This processing shall be completed upon receiving badges. The RSO or designee shall be notified of the results by telephone or email within 48 hours of the vender receiving the badges. Dose Summary Reports The vendor shall maintain occupational dose records of each monitored worker for the current year and lifetime cumulative dose. Two hard copy dose summary reports for each badge series shall be mailed to the RSO each monitoring period. One report shall omit sensitive employee information including; social security number, and birth date. Dose summary reports shall include wear period and the following for each badged worker; o Workers name and badge type o Deep dose equivalent, skin, & eye dose for the wear period, quarter & year o Extremity dose if finger rings are issued Dose summary reports should be received by the RSO within 20 days after the vender receives badges. Vender shall perform Quality Control (QC) on badges including; o Subtracting background radiation measured on control badges o Distinct flag on dose summary reports for irregular exposure or damaged dosimeters Pregnant worker dose summary reports (two reports) shall report the fetal dose and be mailed to the RSO on separate reports for each badged pregnant worker. Annual occupational dose reports - vender shall mail 2 printed annual dose reports (i.e., US NRC Form 5 or equivalent) for each monitored worker per US NRC Regulatory Guide, 8.7, " Instructions for Recording and Reporting Occupational Radiation Dose Data." Reports shall be received by the RSO prior to May 1st. Critical Performance Evaluation Factors: a. NVLAP Laboratory Accreditation for the energy & types of radiation specified in this SOW b. The proposal is to include description the vender's radiation dosimeters, quality control process, timeliness of their past performance, compliance with appropriate regulations, maintaining NAVLAP accreditation, and other information that will assure the Government that your company has the resources to perform the contract requirements should they be selected to perform this contract. (2) Past Performance: Offeror shall have relevant experience in providing the same or similar services. Provide three most recent and relevant references that are likely to respond to our request for information regarding your past performance. References should include company's name, location, e-mail address and point of contact information. The references should be very familiar with your company. The Government may contact the offeror's references for information and may review performance ratings on file for prior Government projects the offeror has performed. Past performance with Government facilities is preferred. Past performance information obtained from sources other than those identified by the offeror may be used.
