SCOPE The Department of Veterans Affairs, Vocational Rehabilitation and Employment Division, Houston Texas Regional Office, is seeking sources for an Indefinite Delivery/Indefinite Quantity contract to provide Independent Living Assessment and Special Adaptive Equipment services to eligible veterans in its Chapter 31 rehabilitation program. SERVICES The services to be provided include: 1) conducting interviews at the veterans place of residence to determine the manner of the veteran's disabilities and the extent to which these disabilities impair their daily living activities; 2) conducting an assessment of the veteran's residence to determine how the space increases the adverse affect of the veteran's disabilities, and how the space might be modified to mitigate those disabilities. 3) Providing a detailed written assessment of the veteran's disabilities; how those disabilities adversely affect their living environment; 4) Providing detailed and specific written recommendations of the special adaptive equipment required, modifications, or other enhancements to the veterans place of residence to enhance the veterans living space; and 5) Furnish and install any or all recommendations made and approved by the Department of Veterans Affairs to include fabrication, modification, or repair of real and personal property. GEOGRAPHIC COVERAGE AREA The Contractor may be required to provide services for the following geographic areas: Houston, Texas Area Counties: Austin, Brazoria, Chambers, Colorado, Fort Bend, Galveston, Grimes, Harris, Houston, Jackson, Lavaca, Liberty, Madison, Matagorda, Montgomery, San Jacinto, Trinity, Walker, Waller, Washington, Wharton San Antonio, Texas Area Counties: Atascosa, Bandera, Bexar, Comal, De Witt, Edwards, Gillespie, Gonzales, Guadalupe, Karnes, Kendall, Kerr, Kinney, McMullen, Medina, Real, Uvalde, Val Verde, Wilson Corpus Christi, Texas Area Counties: Aransas, Bee, Calhoun, Goliad, Jim Wells, Kleberg, Live Oak, Nueces, Refugio, San Patricio, Victoria Rio Grande, Texas Area Counties: Brooks, Cameron, Dimmit, Duval, Frio, Hidalgo, Jim Hogg, Kenedy, La Salle, Maverick, Starr, Webb, Willacy, Zapata, Zavala Northwest Texas Area Counties: Brewster, Crockett, Kimble, Menard, McCulloch, Pecos, Schleicher, Sutton, Terrell Austin, Texas Area Counties: Blanco, Caldwell, Hays, Mason Beaumont, Texas Area Counties: Angelina, Hardin, Jasper, Jefferson, Nacogdoches, Newton, Orange, San Augustine, Shelby, Tyler PERIOD OF PERFORMANCE It is anticipated that any resultant contract will be awarded for a one (1) year base period, and will contain options for four (4) additional one (1) year option periods. ESTIMATED QUANTITIES Based on previous services provided, the estimated number of veterans who will be served under this contract is 250 per year.
