SCOPE OF WORK FOR SERVICE / NEW CO-60 SOURCE
1. This scope of work covers purchasing a new Radioactive Cobalt 60 Source from QSA Global, Inc. and disposing of the old Radioactive Cobalt 60 Source serial number 90189B.
2. Model 680-OP containing model 680B Gamma Ray Exposure Device will be placed in Norfolk Naval Shipyard custom made gray steel over-pack and shipped exclusive use to QSA Global, Inc.
3. QSA shall provide the following services during resourcing model 680B Gamma Ray Exposure Device serial number B249.
4. At completion of service, QSA shall provide the following reports to be provided at time of award :
•· Source Certificate Sheet
•· Wipe Test report (Department of the US Navy wipe test results)
