This is a combined synopsis/solicitation/Notice of Intent for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is NIH-OLAO-OD5-2794774 and the solicitation is being issued as Notice of Intent, under the authority FAR 6.302-1 on a sole basis from Coherent Laser Group.  The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-54 dated November 2, 2011. This acquisition will be processed under Simplified Acquisition Procedures (SAP) and is not a small business set-aside. All responsible sources may submit an offer that will be considered by this agency. The North American Industry Classification System Code (NAICS) for this acquisition is 811310 and the small business size standard is $7.0.  The National Institute of Deafness Communication Disorders intends to procure a twelve (12) month productivity plus service agreement on two (2) Chameleon System: Ultra II for serial number GDP.1195419.2057 and serial number GDP.1117782.4194.  The contractor shall provide covered on all replacement PARTS, including FAP-I's, Labor and Travel for 12 months.  This is a fixed-price contract and the FOB term is Origin and Payment Net 30 days.
