LES is requesting proposals from an independent testing firm (LES’ Testing Representative) to provide material testing and sampling services for the LES Operations Center Project. This scope provides for the material sampling and associated testing outlined in these specifications. The Testing Representative will conduct cast in place concrete, material, structural steel, and masonry inspections. The LES Confidentiality and Security Agreement (Form 790) must be signed and returned by respondent before RFP Attachments B & C are released to them. Please send executed copy to email address:
[email protected] Successful respondent will be required to complete (1) the LES Services/Consulting Contract; 2) furnish proof of insurance with coverage specified in the Contract by an insurance company licensed to do business in Nebraska and rated at least B+ in A.M. Bests Rating. Certificate must show LES listed as additional insured for general liability and include waiver of subrogation; 3) Return a completed copy of the Equal Employment Opportunity Compliance Request Form.
