This is an intention for a sole source, Firm Fixed Price Purchase Order to American Medical Depot, 4380 NW135th Street, Miami, FL 33054. The Laerdal SimMan 3G Manikin, Software and License, and associated equipment with two (2) year warranty will be procured sole source, part #212-01001 with 212-VPLUSP2 and 05-1-100. All responsible offers will be considered, however, offers must be submitted for the item/services listed above. Substitutes are not acceptable. The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-46 (effective 21 September 2010). FAR 52.212-4 and 52.212-5 applies to this acquisition. WAWF procedures also apply. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The NAICS Code is 326199 the size standard is 500 employees. The 711th Human Performance Wing/PK, Brooks City-Base TX intends to award a purchase order by other than full and open competition in accordance with Subpart 6.302-1 (c). Only one responsible source and no other supplies or services will satisfy agency requirements. Include all applicable discounts, shipping, delivery and warranty information for this purchase.
