The purpose of this acquisition is to establish a Standing Quote Agreement (SQA) for five (5) National Stock Number (NSNs), FSC 6515 - Support, Lumbar.  Period of performance will not exceed two years (one year base and a one, 1-year review period) or 150K.  Award will be in the aggregate (all or none) to the offeror who represents the best value to the Government utilizing the two step evaluation process: Step One:  is the determination of technical acceptability. Step 2: Only those quotes that are determined technically acceptable will be evaluated for the past performance and price factors in Step Two.

 FAILURE TO PROVIDE THE SUPPLEMENTAL DOCUMENTATION AS REQUIRED IN Item M #1 of the RFQ MAY RESULT IN YOUR OFFER NO LONGER BEING EVALUATED OR CONSIDERED FOR AWARD

 If responding to this Request for Quotation, complete and return the entire package. pages 1-38 (too include all the necessary documents) by the time specified in block 10 of the RFQ.
