THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND A SEPARATE SOLICITATION WILL NOT BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6 under Simplified Acquisition procedures in Subpart 13.5 as supplemental with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is RFQ162548 and is being issued as a Request for Quote (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-25. This acquisition will be processed under Simplified Acquisition Procedures. The associated North American Industry Classification System (NAICS) Code is 423450. The Government intends to make an award based on the lowest price technical accepted offeror to the eligible, responsible offeror whose offer conforms to the solicitation, and is the most advantageous to the Government. The Government reserves the right to make an award to that offeror whose quotation provides the best overall value to the Government. The Government anticipates a firm-fixed priced contract. The Clinical Center at the National Institutes of Health has a requirement to purchase and install one (1) TeleRehab Advantage telemetry monitoring system in the Rehabilitation Medicine Department that satisfies the accompanying specifications. Minimum Specifications: Four (4) Patient telemetry Transmitters with Receivers (Digital, 608-614 MHz Frequency Range, Pacer Spike Detection) One (1) Advantage Workstation – Windows XP Operating System One (1) 17” LCD Flat Panel Color Monitor with Speakers HIPPA Compliant Software Printer, Network Laser Integrated TeleRehab Outcomes Antenna Kit To allow for sufficient coverage for at least a 30 foot radius around the antenna Set up and Training Warranty parts and labor Four (4) SpO2 Wristband Monitors w/dongle and finger sensors Four (4) MaxFast Forehead Sensors Four (4) SpO2 Sensor Adaptors One (1) Handheld PDA Software ability to allow exporting of trending data into Excel or ASCII format The detail Statement of Work can be found in Attachment No. 1 to this synopsis.
