The agreement is for various levels of services explained in the Services Agreement, Quote number 20517595 / -2. The agreement is for extended warranty plus coverage, which includes all labor and parts (excluding consumable necessary to restore the instrument to operating specifications. Repairs may be made on-site, at Shimadzu's Services Center, or a replacement may be provided.

In addition, one premium preventative maintenance visit has been included. The premium preventative maintenance visit shall include necessary cleaning, adjustments, verifications, and parts replacement to ensure OFTS's instruments are properly serviced and performance verified for accreditation purposes and in meeting forensic drug testing standards of practice.

Contractor shall review Attachment I of this document, the Statement of Work, for more information.
