I. Proposal Requirements

Project: To supply water treatment services for water cooled heat pump system in the cafeteria.

Lowest price will not guarantee contract award. Contract award will be based on quality, experience, expertise and price.

Payment: Payment Terms are Net 30 days of completion and receipt of Invoice.

Site Visit Date: To be scheduled.

a) Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # GL2013-006 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same.
II. Project Description

Scope of Project:
• To supply water treatment services for the cafeteria water cooled heat pump system monthly.


III Specific Requirements:

• The contractor shall provide water treatment services for water cooling heat pump system in the cafeteria on a monthly basis.
• The contractor shall inspect and adjust the water treatment equipment as necessary.
• The contractor shall maintain proper chemical concentration.
• The contractor shall collect and analyze water samples and submit water analysis report as required to the Center Maintenance Manager.
• The contractor shall service as needed all water treatment equipment defined as, chemical feed tanks, by pass feeder, chemical and biocide injection pumps, solenoid valves, control panels, time clocks, agitators and water meters.
• The contractor shall inform the Maintenance Manager of any equipment failure that would stop or delay the required water treatment.


Note: Please inform the Welcome Center that of your delivery. Please drop off all invoices in finance, room number 13.

 

IV Center Requirements

Center Regulations:
Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements.


Management of Jobsite:
The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day.

The Contractor shall clear all deliveries with the Center's security personnel.

 


Insurance:
The Contractor shall obtain and maintain insurance as follows:

Workman's Compensation/Employers Liability insurance, to meet New York statutory requirements

Comprehensive General Liability ($1,000,000 per occurrence)

Comprehensive Automobile Liability ($1,000,000/combined single limit)

Fire Legal Liability ($500,000 per occurrence)

The Contractor must cover any deductible costs. Upon contract award, the Contractor must add Glenmont Job Corps Center as additional named insured to the policies.

Schedule:
Monthly from September 1, 2013 to December 31, 2013.

V. Proposal Form

The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID # GL2013-006 and address all items in sections II & III, to be considered.

Bids must to be valid for the entire period.

The Contractor shall submit the following information with the proposal:


References

Copies of Insurance Certificates and applicable licenses.
Completed vendor questionnaire and W9.
