Statement of Work
Introduction
The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the vendor in supplying web-based residency tracking services, as required by the Accreditation Council for Graduate Medical Education (ACGME) for ACOS/E and/or designated users of the Department of Veterans Affairs, Greater Los Angeles Healthcare System (VAGLAHS), located at 11301 Wilshire Blvd, Los Angeles, CA 90073.
2. Scope
The vendor shall provide the following services to ACOS/E and/or designated users for 1 year (July 1, 2016 - June 30, 2017). Program must be acceptable by UCLA or, a program UCLA already utilize.
Access to web-based residency tracking program and all relevant updates or new releases
Support as required by the Office of Associate Chief of Staff/Education
3. Implementation
N/A - implementation of service began in 2014.
4. Period of Performance/Acceptance
The period of performance for the contract requested is from July 1, 2016 through June 30, 2017.
5. Vendor Responsibilities
Indicate location of performance - Office of ACOS/Education, Building 218, Room 217
Specific deliverables: web-based program that collects manages, maintains datasets associated with the activities of our Trainees and is able to compile reports based on the datasets.
Problems/unforeseen situations will be handled via person to person telephone calls, conference calls, e-mail and e-conferences.
Resources to be provided by vendor include support personnel (both technical and training) as well as program maintenance in the form of periodic updates, as they are released.]
6. Government Responsibilities
The government will provide a Project Manager.
7. Hours of Operation
Though the Department is open 24/7, the normal tour-of-duty hours at VHAGLA is 7:30 am to 5:00 pm, Monday through Friday, excluding Federal holidays. There may be instances in which the vendor's workday may be outside of this period in regards to instrumentation issues. However, the ordering and delivering of supplies will be within the time frame of a normal tour.
In accordance with 5 U.S.C. 6103, Executive Order 11582 and Public Law 94-97 the following national holidays are observed and for the purpose of this contract are defined as "Legal Federal official holidays":

When a holiday falls on a Sunday, the following Monday will be observed as a National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by U.S. Government Agencies.
