VA69D-14-I-0381; Source Sought Notice for Milwaukee Ambulance Service Jessica L. Howard, Contract Specialist, NCO 12 GLAC 1/16/2014 The Clement J. Zablocki VA Hospital, 5000 W National Ave, Milwaukee, WI 53214 is seeking potential sources capable of providing all personnel, equipment, and material necessary to provide for emergent and non-emergent, basic and advanced life support ambulance service, twenty-four hours per day, 365 days per year, on an as needed basis. These services include advanced life support, critical care support, and basic life support ambulance transportation for the Clement J. Zablocki VA Hospital. Transportation may include but is not limited to one way trips between VA facilities, resident to VA facility, and VA to affiliated hospitals. Contractor should have the capability to provide a critical care level of service including respiratory ventilator transportation, on staff nurse or equivalent, and all associated labor, equipment, and supplies necessary to assure patient safety in process. Emergency Medical Technicians (EMT) providing emergency service on ambulances under this contract must have completed training in accordance with the standards published by the Department of Health and Human Services . EMTs shall be certified, licensed or otherwise officially recognized by the local, state or regional government or public entity where the emergency ambulance service is operated or by which it is governed. Ambulance drivers shall have a valid operator's or chauffeur's license in accordance with Federal, State and local government requirements for their place of operation, for the services they perform and be capable of administering oxygen. The applicable NAICS Code is 621910 with a size standard of $14.0Million. No formal solicitation document exists at this time. Responses to this sources sought notice should include the following: name and address of company; business size (large or small); identify whether or not firm falls into any of the following socioeconomic categories - 8(a), HUBZone, Service Disabled Veteran-Owned (certified on VIP); firm's experience in providing emergent and non-emergent, basic and advanced life support ambulance service in a professional setting and any other pertinent information, which demonstrates ability to meet the above requirement.
