THIS IS NOT A REQUEST FOR PROPOSAL OR A REQUEST FOR QUOTE. IT IS STRICTLY A REQUEST FOR INFORMATION (RFI). UNSOLICITED PROPOSALS WILL NOT BE CONSIDERED IN RESPONSE TO THIS RFI. NO CONTRACT WILL BE AWARDED PURSUANT TO THIS ANNOUNCEMENT.
1. SUBJECT: The 773rd Enterprise Sourcing Squadron (ESS), Contracting Office is requesting information on certified HubZone business concerns under NAICS code 811213 capable of providing Video Teleconferencing (VTC) Maintenance. Responses/questions concerning this request for information (RFI) are due by 1:00 P.M, Central Standard Time (CST) on 24 Apr 13. Any potential vendor offering services under the applicable NAICS code may submit responses/questions to this RFI.
2. DISCLAIMER: This RFI is issued solely for information and planning purposes and does not constitute a solicitation. Responses to this notice are not offers and will not be accepted by the Government to form a binding contract. Respondents are solely responsible for all expenses associated with responding to this RFI. All information received in response to this RFI marked as "proprietary" will be handled accordingly. Information provided as a response to the RFI will not be returned. If a solicitation is released, it will be synopsized and posted on the Federal Business Opportunities (FedBizOpps) website located at https://www.fbo.gov. . It is the sole responsibility of the vendor to monitor FedBizOpps for additional information pertaining to this RFI and any future requirements.
3. BACKGROUND: Air Force Medical Services is collecting information on federally certified platinum provider offering Video Teleconferencing (VTC) Maintenance. Respondents shall provide system maintenance warranty and technical support for Polycom HDX 7001 Video Teleconferencing (VTC) system supporting the AFMS wounded warrior telemental health program, IAW Statement of Work dated 14 March 2013.
4. PERIOD OF PERFORMANCE: The proposed period of performance for this effort is for twelve (12) months (one year) after date of award, and two (2) option years (twelve months each).
5. REQUIREMENTS: Vendor shall provide:
a. Advance Parts Replacement - provide advance replacement for any failed hardware component covered under the Premier Service program. All parts will be shipped by Polycom for advance replacement using an expedited carrier service. All shipping charges to/from the site will be paid by the vendor.
b. Software - provide DoD approved system updates and upgrades at no additional charge.
c. Technical telephone support - provide technical telephone support to assist with diagnosing, configuring, and troubleshooting HDX 7001 VTC system.
d. Support portal - provide access to Polycom online 24/7 portal for registration, license lookup, knowledgebase, product document and software download, on-line service requests and status checking, and RMA status checking and delivery tracking information.
e. See attached final Statement of Work (dated 14 Mar 2013) for more details on this new requirement.
6. RESPONSE INFORMATION: Response to this RFI must be submitted to Mr. Apolinar Vargas, Contract Specialist, via e-mail to [email protected]. Email responses should be no larger than 10 megabytes (MB) in size. If larger attachments are required, please notify 773 ESS at the above listed email address. Each RFI response will receive a confirmation email; if a confirmation is not received please call 210-395-9843. Please include a point of contact, phone number, e-mail address, website information, Cage Code, DUNS number, business size and whether the company is a foreign or domestic entity.
Any questions relating to the RFI shall be submitted to the Contract Specialist and Contracting Officer by 8:00 AM on 19 Apr 2013. Any questions received after this time/date may or not be answered due to time constraints of the RFI.
Responses must be received by this office no later than 1:00 P.M, Central Standard Time (CST) on 24 Apr 13.
This RFI is not a commercial solicitation. The Government will not pay for any information submitted, or for any costs associated with providing the information.
The response should describe the company's capability to offer and certify Video Teleconferencing (VTC) Maintenance as characterized by the requirements addressed in this RFI.
7. LATE SUBMISSIONS: Failure to respond to this RFI does not preclude participation in any future competition, nor will information provided in response to this RFI be used to exclude anyone from responding to any future requests for proposals. Communications with 773 ESS in regard to this RFI will only be permitted in writing during the RFI response period.
The Government does not guarantee that questions or responses to the RFI received after the submittal deadline will be considered.
8. OWNERSHIP OF RESPONSE TO RFI: All informational material submitted in response to this request becomes property of 773 ESS.
9. RELEASE OF CLAIMS, LIABILITY, AND PREPARATION EXPENSES: Under no circumstances shall 773 ESS be responsible for any response preparation expenses, submission costs, or any other expenses, costs, or damages of whatever nature incurred as a result of the respondent's participation in this RFI process. Respondent understands and agrees that they submit a response at their own risk and expense, and release 773 ESS from any claim for damages or other liability arising out of the RFI process.
10. ERRORS IN RESPONSE: 773 ESS shall not be liable for any errors in respondent's response. Respondent is responsible for careful review of its entire response to ensure that all information is correct and complete. Respondents are liable for all errors or omissions contained in their responses.
11. ADDENDUM: 773 ESS reserves the right to issue an addendum to this RFI at any time for any reason.
