This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under FAR Part 13, Simplified Acquisitions and FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)) and FAR Part 37.101, Non-Personal Services. This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Billings Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Quotation (RFQ)-09-15-010-PRL. This solicitation is issued on an unrestricted basis. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-83. The associated North American Industry Classification System code is 562112 and the small business size standard is $38.5 million.

INTRODUCTION: This requirement is for pickup and disposal services for solid waste at the Wind River Service Unit (WRSU) at Fort Washakie and Arapahoe Wyoming.

2. PERIOD OF PERFORMANCE
Performance of the contract shall be from October 12, 2015 through September 30, 2016, with four 12 month options.

If the Government exercises the options pursuant to Federal Acquisition Regulation (FAR) 52.217-9, Option to Extend the Term of the Contract (MAR 2000), the period of performance shall be extended in accordance with the following schedule:

Year Option Period
Option Year One (1) to be completed by the Government at time of award.
Option Year Two (2) to be completed by the Government at time of award.
Option Year Three (3) to be completed by the Government at time of award.
Option Year Four (4) to be completed by the Government at time of award.

3. PURPOSE OF THE CONTRACT
The purpose of this acquisition is to contract for Solid Waste Collection Service for the Wind River Service Unit, PHS Indian Health Centers, Arapahoe and Fort Washakie, Wyoming. The schedule is to pick up weekly and will be coordinated with the Facility Manager.

4. STATEMENT OF WORK / MAJOR DUTIES
The Contractor shall furnish solid waste pick-up and removal services for the Wind River Service Unit, separate locations; The PHS Arapahoe Health Center (#14 Great Plaines Rd. Arapahoe Wyoming 82510) and the PHS Ft. Washakie Health Center (Bldg. #29 Black Cole Rd. Ft. Washakie Wyoming 82514) and Nine (9) Residential, (#74 1st St. O.E.H. Office, #75 1st St. Supply Office, #76 Black Cole Business Office, #77 Black Cole, #78 Black Cole, #79 Sacajawea Cir., #80 Sacajawea Cir., #81 Sacajawea Cir., #82 Sacajawea Cir.) all with in the Ft. Washakie Complex.
