Purpose
This request is for the Annual Service Agreement on the Ziehm Solo instrument, serial# 51607. This equipment is used to provide fluoroscopic imaging support for Clinical Center patients in the Intensive Care Unit and for the Procedure Vascular Access Conscious Sedation Service (PVCS). Maintenance contracts on patient care equipment are necessary to meet the Joint Commission's patient safety initiatives.
Scope
The Offeror shall provide the following:
The vendor will provide the following services under this service agreement for the Ziehm Solo, Serial number 51607:
• On Site Service Coverage, Monday-Friday from 8am to 5pm (excluding Holidays and Weekends).
• Next day service if needed
• 24 hr US Service Call Center
• One Preventive Maintenance (PM) per year during contract hours.
• Service may be performed by Ziehm Service Technician or an Authorized Service Provider.
• Covers Glassware
• Full coverage of the systems l.l., X-Ray Tube and Monitors.
• After-hours service available at standard hourly rate.
• Documentation for all services, parts used and work performed at the time service has been completed.

Required Performance Capabilities:
Information will be collected from Small or Large Manufactures and authorized distributors at this time to determine if the requirement can be met using a small business set-aside. All respondents are required to certify if they currently meet the small business requirements under the North American Industry Classification System (NAICS) code [811219], Other Electronic and Precision Equipment Repair and Maintenance with a standard size of $20.5M, as of the date of their submittal.
CAPABILITY STATEMENT - Any potential Manufacturer must demonstrate the following:
1) The offeror shall submit product literature for the supplies it is proposing. Capability Statement(s) should clearly identify the following:
a. Name of the Manufacturer, quantity and location where the units are produced (domestic and/or international);
b. A signed statement to state, "We are the Manufacturer of the items and/or we are authorized distributors of company X";
c. Estimated Number of days, after receipt of order that is typical for delivery of such items;
d. Any other relevant information that is not listed above which the Government should consider in developing its minimum specifications and finalizing its market research.
2) Provide the following business information:
a. DUNS number
b. Company Name
c. Company Address (including point of contact, phone number and email address)
d. Type/Size of company, NAICS code as validated in SAMS.
Respondents will NOT be notified of the results of the evaluation.
