LES is requesting proposals from qualified full service professional custodial service companies to provide Custodial Services in a mixed environment of office space and shop space at multiple facilities in and around Lincoln Nebraska. [email protected]** Prior to LES providing the detailed information vendors will be required to complete the attached LES Form 790 Confidentiality and Security Agreement. Successful respondent will be required to complete (1) the LES Services Contract; 2) furnish proof of insurance with coverage specified in the Contract by an insurance company licensed to do business in Nebraska and rated at least B+ in A.M. Bests Rating. Certificate must show LES listed as additional insured for general liability and include waiver of subrogation; 3) Return a completed copy of the Equal Employment Opportunity Compliance Request Form; 4) Complete LES Form F790 (Confidentiality & Security Agreement).
